This article discusses the barriers that transgender individuals face when seeking healthcare and proposes a model to improve access for transgender adults. The article highlights that many transgender patients experience discrimination, lack of insurance, financial instability, and a lack of knowledgeable providers as barriers to receiving adequate care. The authors argue that the lack of provider knowledge is due to attribution bias and trans erasure at the informational and institutional levels. They propose an integrated model of care delivered by primary care physicians who provide routine gender-affirming treatments and other preventative health care. Two successful variations of this model are described in Boston. The article emphasizes the importance of primary care in transgender health and highlights existing programs in Boston and New Zealand that provide comprehensive care for transgender patients.
