A recent study conducted in Saudi Arabia compared the accuracy of vancomycin dosing in elderly patients using actual serum creatinine (SCr) levels versus rounded SCr levels. The study found that dosing based on actual SCr levels was more accurate and correlated better with the targeted maintenance dose (TMD) of vancomycin compared to dosing based on rounded SCr levels. Dosing based on actual SCr levels also had a lower error percentage and higher accuracy rate within 10% of the TMD. The study concluded that estimating vancomycin dosing based on actual SCr levels is more accurate and can prevent under-dosing of the medication. [Extracted from the article] |
