Hypertension and cardiovascular disease may represent a major association between these conditions. Angiotensinogen (AGT) gene The M235T polymorphism is associated with an increased risk of hypertension. A missense mutation in the exon two genes (AGT M235T) has been associated with increased AGT levels, with 235TT homozygotes having between 10% and 20% more plasma AGT than 235MM individuals. The frequency of the 235T allele is significantly greater in hypertensive patients than in normotensive. Nitric Oxide (NO) is reported to have a major role in regulating blood pressure, and impaired NO bioactivity is an essential component of hypertension. Model studies in mice with impaired genes for endothelial NO synthase have high blood pressure levels compared to control animals. It suggests that a genetic component is related to damaged NO bioactivity and hypertension. Hypertension shows a dilator response that implicates NO synthesis, which causes endothelial cell disruption. In-vitro studies suggest that endothelial cells cause acetylcholine to induce vasodilation by releasing relaxation factors related to NO. The mechanism synthesizes nitric oxide from L-arginine uses a different enzyme, constitutively active and subsequently activated by endothelial shear stress. After synthesis, some of the NO will rapidly diffuse into the underlying vascular smooth muscle (NO is fat and water-soluble gas), whereas that released into the lumen is oxidized quickly by superoxide anions (to peroxynitrite and then nitrate) and heme hemoglobin. (to form nitrate and methemoglobin). Endothelin-1 (ET-1) is a vasoconstrictor secreted by endothelial cells, which acts as a natural partner of the vasodilator nitric oxide (NO).
