This article discusses the importance of accurate documentation of in-hospital cardiac arrests (IHCA) in electronic medical records (EMRs). The authors highlight the challenges faced by hospitals without built-in cardiac arrest note templates in their EMRs, which can lead to gaps in crucial arrest information and hinder timely patient care. To address this issue, the authors developed a quality improvement initiative that included the development of a standardized code blue note template based on Utstein-style guidelines. The implementation of this template resulted in a significant improvement in the quality of cardiac arrest documentation and post-return of spontaneous circulation (ROSC) care. The authors emphasize the importance of including core variables in the documentation to maintain quality and suggest further exploration of the impact of incorporating post-ROSC care in arrest documentation.
