A 20-year-old man with no past medical history presented to the emergency department of a level 1 trauma centre after sustaining significant pelvic trauma. The left testicle left testicle demonstrates a normal echotexture and internal vascularity. Despite the low clinical suspicion for testicular rupture at presentation, the patient underwent early ultrasonography which reported an intact but avascular right testicle.
