Calciphylaxis, also referred to as calcific uremic arteriolopathy, is a rare and often fatal disorder that is best known for causing cutaneous ischemia and subsequent necrosis due to calcification ot arterial vessels in the underlying dermis and adipose tissue. Although calciphylaxis primarily occurs in patients with end-stage kidney disease who are receiving dialysis, it can also present in patients without endstage kidney disease and in patients who have undergone kidney transplantation and is accordingly termed nonuremic calciphylaxis. Nonuremic calciphylaxis demonstrates pathologic morphology like calcific uremic calciphylaxis and has a similar mortality rate, as high as 52% (Clin J Am So c Nephrol. 2008;3(4); 1139-1143). However, nonuremic calciphylaxis is not as thoroughly detailed as calcificuremic calciphylaxis in the literature, making its diagnosis and management more difficult for providers. Our patient is a 65-year-old Caucasian female w'ho presents to die hospital with encephalopathy stemming from sepsis. Her sepsis originated from infection of chronic ulcers present on her left lower leg due to calciphylaxis. Management of her calciphylaxis is complicated by her co morbidities, and treatment is debated. [ABSTRACT FROM AUTHOR] |
