Intravascular papillary endothelial hyperplasia in benign fibrous histiocytoma.

Abstrakt: A 64-year-old man presented with a 10-year history of exophytic, polypoid, painful tumoration in the right leg. Physical examination revealed a soft, 2.5-cm violaceous, hemorrhagic-like lesion, unattached to deeper structures. Following the initial clinical diagnosis of vascular tumour, the lesion was completely excised. Histologically, within the dermis and hypodermis a tumoral lesion was seen, consisting of macrophage like cells with cytoplasmic lipidic vacuoles. Some of the cells were Touton-like in appearance and had a vesicular nucleus. In the present case, the double immunological phenotype (histiocytic and endothehal), seen in the endothelial papillary growth would confirm the reactive nature of the lesion. This suggests the possibility of an endothelial histiocytic origin.
