Background: Insulin autoimune syndrome (IAS), also known as Hirata’s disease, is a rare cause of spontaneous hyperinsulinemic hypoglicemia characterised by the presence of autoantibodies directed against human insulin (IAA).

Insulin autoimmune syndrome (IAS) is an immune-mediated hyperinsulinaemic hypoglycemia in individuals previously unexposed to exogenous insulin. A 63-year-old woman developed recurrent hypoglycemic episodes despite being on optimal glycemic support.

Insulin autoimmune syndrome or Hirata’s disease is an extremely rare condition leading to hypoglycaemia of variable severity due to autoantibodies against insulin. We present the first case documented in the Czech Republic.
